Please complete this form to start the registration process with Student Disability Services (SDS). After you have submitted this form and uploaded supporting documentation of your diagnoses, you will need to schedule an Accommodations Meeting. During this meeting, we will discuss your accommodation request, review your documentation, and determine the next steps. Your accommodations request cannot be processed or approved until the intake and registration process is completed. Accommodations meetings should be scheduled after students have completed this intake and shared any related supporting documentation. Accommodations request cannot be processed until both steps are complete.

Documentation should be provided by a licensed or otherwise properly credentialed professional who has appropriate and comprehensive training, relevant experience, and no personal relationship with the individual being evaluated. Additionally, the professional diagnosing a disability should be an appropriately trained evaluator/professional, such as a physician, psychologist, psychiatrist, or educational diagnostician.  For example, an audiologist would diagnose a hearing impairment; a psychiatrist, psychologist, or clinical social worker would diagnose a mental health disability.  Documentation from a family member or family friend cannot be accepted.

Additional Information

Student Disability Services will only discuss your academic accommodations with faculty or staff of Spartanburg Community College who have a legitimate academic interest, as defined by the Family Educational Privacy Rights Act (FERPA).  Medical documentation will be kept strictly confidential and only be viewed by employees of Student Disability Services, administrators of Spartanburg Community College, and regulatory agencies in the course of legitimate inquiries.  Spartanburg Community College does not collect genetic information about students. SCC considers supporting documentation confidential, and thus the information does not become part of a student’s academic transcript. All disability-related files are maintained following the SC Department of Archives and History Records Retention Schedule. Student Disability Services recommends that every student keep a copy of their supporting documentation. Disability documentation can only be released in accordance with the law and cannot be provided to anyone other than the student. Students may access, print, and save a copy of their files when needed through the department's accommodations management platform. SDS may provide a verification letter of registration status and approved accommodations at the student's request.
